The fee for this service is $625 each resale (unless RUSH was requested-add $100). Once payment is received in our office, we will fill your order. Our typical turn-around time for this documentation is ten (10) business days once payment is received in our office. If needed within three (3) business days of payment, the cost will be an additional RUSH fee of $100 per request.*
*CAM will process the request once the payment AND form are received.
